You may pay your for your membership using PayPal below, or by enclosing the executed document with a check and mailing to Local 104, AFM/515 E 4500 S/Ste 100/Salt Lake City, UT/84107-4530
Important Note
Before initiating this payment process, in your browser menu, go to the TOOLS tab; select INTERNET OPTIONS; in the SECURITY tab, choose “DEFAULT LEVEL”; in the PRIVACY tab, lower the settings calibration until the message “ACCEPT ALL COOKIES” is displayed. Make sure that the POP UP BLOCKER option is NOT checked. After you have made these alterations, close your browser and re-open it. Access www.utahmusicians.org and proceed with your payment. Selecting any of the radio buttons below will afford you the option of paying by EITHER a credit card or through your PayPal account. If you choose to pay using PayPal, you must type YOUR OWN e-mail address and password in the appropriate fields. Our e-mail address (afmslc@integra.net) may appear in the e-mail address field by default.
